We Manage Chronic Conditions

Hypertension

Hypertension is a common condition where the force of blood against the artery walls is consistently too high. This can lead to serious health issues, including heart disease and stroke. If you have high blood pressure, the force of the blood pushing against the artery walls is consistently too high. The heart has to work harder to pump blood.  Untreated, high blood pressure increases the risk of heart attack, stroke and other serious health problems.  Healthy lifestyle habits —such as not smoking, exercising and eating well — can help prevent and treat high blood pressure. Some people need medicine to treat high blood pressure.

Diabetes Mellitus

Diabetes mellitus is a condition defined by persistently high levels of sugar (glucose) in the blood.  There are several types of diabetes.   The two most common are called type 1 diabetes and type 2 diabetes. Type 1 diabetes is an autoimmune disease. This means it begins when the body's immune system mistakenly attacks other cells in the body.   In type 1 diabetes, the immune system destroys the insulin-producing cells (called beta cells) in the pancreas. This leaves the person with little or no insulin in his or her body.  Type 2 diabetes is also called adult-onset diabetes.   That's because it almost always used to start in middle or late adulthood.   However, more and more children and teens are now developing this condition.

High Cholesterol

Hyperlipidemia (high cholesterol) is an excess of lipids or fats in your blood. This can increase your risk of heart attack and stroke because blood can’t flow through your arteries easily.   High levels of cholesterol in the blood may lead to a buildup of plaque in your blood vessels, particularly arteries.  As plaque builds up in an artery, it gradually reduces the amount of blood that can flow through that artery.  As a result, some organs may not receive enough blood. Over time, this can result in permanent damage to those organs.Adding exercise and healthy foods can lower your cholesterol.  Some people need medication, as well. Managing your cholesterol is a long-term effort.
